One of our staff submitted her medical leave form but there was no medical leave chit but a time off slip from the hospital showing proof that she had been in the hospital for treatment from 8.30 am till 12.30 p.m.  Our working hours are from 8.30 am to 5.00 pm.  As such, can this time slip be considered as medical leave?

Time off slips from hospital are not medical sick leave certificates. If the company does not provide time off for employees to see doctors and do not treat time off slip as MC, let the employees know about it so there will not be confusion.

Time slip shows from 8.30 am to 12.30 pm.  Company is agreeable with this.  However, staff must report back to work by 12.30 or 1.00 pm but she did not turn turn up the whole day and submits the time slip as MC.  Is this allowed as per labour law?

Then you can  treat it as one day annual leave. Let that employee know about it as she might be thinking that the slip could be used for sick leave for the day.
